Your Privacy

First Avenue Family Practice values the privacy of all our patients. No patient information is given to a third party without your consent.

Our staff are trained in patient confidentiality and have signed a “confidentiality of information” contract.

Your patient records are fully computerised and are only accessible by the doctors and nurses.

Communication policy

Messages can be sent through to the practice email at telehealth@fafp.com.au. These will be reviewed each day and either responded to in 1-2 business days or passed on to the relevant doctor as required.

We do not take bookings through email, please call the practice on 9713 5869 or book online using HotDoc

Receiving the results of any test or procedure

If you have had a test done, please contract the surgery 3-5 business days following the test for the results, or advised by your doctor .

If is our policy NOT to notify patients if an appointment with the Doctor is NOT required following tests

My Health Record

Our doctors can help you to create a secure medical summary and decide what information you would like to upload to your My Health Record. We encourage you to make a visit to do this to ensure that information is correct, up to date and what you would like to have there.

Complaints

Please talk to your GP or the practice manager if you have any complaints or suggestions. Alternatively you may use the feedback boxes which are located in the waiting rooms. If you have a complaint about your care that is not resolved in this way, you may contact the Health Care Complaints Commission.
